The following items are acceptable forms of enrollment:

Public/Private School students:  Report Card from the previous school year OR 3rd/4th grade Progress Report/Report Card from current school year.
Homeschool students: letter from the Department of Education stating student is homeschooled along with a copy of student's birth certificate.
